At CABS Health Network, we provide Enhanced Health-Related Social Needs (HRSN) Care Management to help Medicaid members access critical resources and social support services. Our approach ensures that members receive personalized outreach, referrals, and ongoing care coordination to improve health outcomes and overall well-being.
Our Care Management Services Include:
Medicaid members receiving Enhanced HRSN Services may qualify for transportation to:
- Outreach & Engagement – Identifying and connecting Medicaid members to essential services.
- Eligibility Assessment – Determining a member’s qualification for Enhanced HRSN services.
- Referral Management – Assisting with access to housing, food assistance, employment support, and healthcare services.
- Social Care Plan Development – Creating a personalized care roadmap tailored to each member’s needs.
- Care Coordination & Follow-Up – Ensuring members receive and benefit from assigned services.
- Education & Support – Providing guidance on navigating social care services and healthcare systems.
- Application Fee Assistance – Covering costs related to housing applications, background checks, and other necessary fees.
