The Agency is currently launching a Community Wellness Initiative with Family Certified Care Services (FCCS) to educate and train residents in low-income housing facilities. The team will help these residents overcome social determinants such as access to health services, social support, environmental conditions, and nutrition. By providing a comprehensive, coordinated care plan, the duo aims to reduce unnecessary hospitalization and improve community health and wellness outcomes.
