We deliver services leveraging a value-based care approach that provides services and always gives caregivers and patients the forum to provide feedback to ensure continued service improvement.
Ongoing + Realtime Assessments – CABSHN case coordinators (“CC”) and home care workers consistently assess our client’s condition, response to treatment, and their daily state of health.
Track Changes in Clients’ Behaviors – Caregivers and CC’s are the first lines of defense. Caregivers are trained to contact their supervisors to report any changes in their client’s condition, unusual behavior, change in eating habits, speech, or consciousness.
Prompt Reporting of Unusual Incidences – Our case coordinators are proactive in reporting incidences to the Director of Patient Services and the field nursing staff for further direction. Our caregivers have 24/7 support and guidance: providing real-time client care.
Engage Help + Limit Unfavorable Escalation – CABSHN nurses perform wellness visits when indicated. This ensures health insurance plans are both contacted and engaged in preventive measures. This has been effective in reducing hospitalizations and the advancement of chronic diseases.
Payment Sources for Care
You can pay for services through many options:
We assist clients in applying for:
- Home health aide or personal care services from Managed Long-term Care (MLTC) and Managed Care Organization (MCO)
- Medicaid if they qualify automatically
- Medicaid when there are income eligibility issues. In these cases, CABS works with our partners to complete Supplemental Needs Trust (SNT) for these clients to get them qualified for Medicaid
- For clients with just Medicare, CABS’ can assist the client in obtaining short-term care through our CHHA partners
